Computer-assisted provision of hormonal contraception

Project abstract

This study will evaluate a novel means of improving access to family planning services for women with limited contact with the health care system.

Using a randomized controlled design, we will assess the impact of using a computer kiosk to increase provision of contraception in 3 clinical settings (emergency departments, safety-net clinics, and retail clinics) which serve patients who have little other access to health care. These computer kiosks are designed for patients with minimal computer skills and low literacy and are currently being used to facilitate care of women with urinary tract infections (with funding from the California HealthCare Foundation). We propose to concurrently use these kiosks to assess women's need for contraception, screen for contraindications to hormonal contraception, and print out prescriptions to be signed by a clinician.

Randomization will be by day: on intervention days, women will be offered computer-assisted provision of hormonal contraception; on control days, the kiosks will be used for computer-assisted treatment of urinary tract infections.

Our primary outcomes will be the proportion of women of reproductive age who leave the clinic with a prescription for hormonal contraception (assessed by chart review). Secondary outcomes will include women's satisfaction with the system (assessed by a computerized survey administered immediately following use of the kiosk) and use of contraception 3 months after visiting the clinic (assessed via a follow up phone interview).

This project will bring together an accomplished multi-disciplinary team with a history of successful collaboration that has previously demonstrated the efficacy of computer-assisted provision of emergency contraception and management of urinary tract infections. The proposed study is an ancillary study to an already funded study evaluating computer-assisted treatment of urinary tract infections.
