Client and provider perspectives on reproductive intention screening during the primary care encounter: using a reproductive justice framework to strengthen reproductive health services in primary care in New York Phase II

Project abstract

Insufficient formative research has been done to design best practice around inclusion of reproductive intention screening in primary care encounters. To fill this research gap, this study will assess client preferences and provider perceptions of reproductive intention screening, sexual health discussions, and contraceptive decision-making during primary care visits in New York State (NYS), as well as to test whether these preferences and perspectives differ by socio-demographic and clinic characteristics. To achieve these goals, we will conduct an anonymous cross-sectional waiting room survey of approximately 1,080 clients at four primary care clinics (two in NY City and two in Putnam County) and an internet-based survey of approximately 450 family practice and internal medicine providers throughout NYS.

The proposed research is innovative in its participatory approach, begun in Phase I, to developing practice recommendations in collaboration with clients, providers, reproductive justice advocates and other key stakeholders in NYS. This type of participatory work strengthens the likelihood of success for improving health outcomes. Measuring providers' gaps in knowledge, comfort with reproductive intention screening and subsequent counseling, and potential biases is important for ensuring provision of these services within a reproductive justice framework that promotes autonomy, regardless of an individual's socio-demographic characteristics. Understanding clients' preferences and perceptions around wording of reproductive intention questions and decision-making reduces the likelihood for ostracizing subpopulations. The results from this study will be used to strengthen the integration of reproductive health services into primary care in NYS currently underway and to inform future research.
